Use of this Service Provider Section is subject to the following

All terms of our General
Conditions of Use are incorporated into this Service Provider
Section Conditions of Use as if specifically incorporated. |

| 2. |
You undertake to keep your password secret
and confidential from unauthorised persons at all times. If you suspect
that the confidentiality of your password has been compromised, you
are required to notify us immediately so that your password can be
changed. |

| 3. |
Notwithstanding anything contained elsewhere
in these Service Provider Conditions of Use or elsewhere on the Sizwe
website, Sizwe shall not be liable for any loss, damage or costs arising
from the loss of confidentiality, use, abuse or misuse of your password
by you or any third party. |

| 4. |
All reasonable steps have been taken to
ensure the accuracy of information contained in this Service Provider
Section. However, Sizwe shall not be liable for any loss, damage or
costs which arise from the use of this Service Provider Section including
not limited to inaccuracies in any information contained in this Service

The information which you are able to access
in this Service Provider Section is confidential. Accordingly, you
are required to keep all such information strictly confidential and

EMPLOYER SECTION
CONDITIONS OF USE
In this section, you are able to download,
complete and electronically submit a membership application form and you
can view your statements of account for any specific month. Use of this
section is subject to the following terms and conditions

into these Employer Section Conditions of Use as if specifically incorporated. |

| 2. |
You undertake to
keep your password secret and confidential from unauthorised persons
at all times. If you suspect that the confidentiality of your password
has been compromised, you are required to notify us immediately so
that your password can be changed. |

| 3. |
Not withstanding
anything contained elsewhere in these Employer Conditions of Use or
elsewhere on the MSP Sizwe Medical Sceheme ("Sizwe") website,
Sizwe shall not be liable for any loss, damage or costs arising from
the use of the Employer Section including, but not limited to, through
the loss of confidentiality, use, abuse or misuse of your password
by you or any third party or inaccuracies in any information contained
in this Employer Section. |

| 4. |
The membership application
form may be completed and submitted electronically, however, a paper
based copy of the completed form, signed by a duly authorised person,
must be delivered and reach Sizwe within five (5) days of date of
electronic transmission of the form. |

|
| 5. |
If the form is not
received within the aforementioned five day period, membership activation
may be delayed. |
| |
|
| 6. |
The paper based version
of the membership application form is required to conform precisely
to the electronic version. In the event of any discrepancy between
the two, the paper based version shall prevail. |
| |
|
| 7. |
The conditions of
your employees' membership of Sizwe will be those applicable at the
time of electronic submission of the membership application form.
Sizwe does, however, reserve the right to amend these conditions on
receipt of the paper based membership application form, whether or
not there are any differences between the electronic and paper based
forms. |

|
| 8. |
All statements of
account that may be viewed in this Employer's Section are for information
purposes only, their accuracy is not warranted and only the original
paper based statement of account which you receive each month should
be relied or acted upon. Sizwe accepts no liability of any sort, howsoever,
arising from reliance being placed upon any statement of account preparing
in this Employer Section. |
